CLIENTS

The paperwork listed below serves two purposes. First, it will inform you about confidentiality, insurance, fees, and other relevant information. Second, certain steps can be printed so that our first session will be as productive as possible.

I hope you will find the information contained within this section helpful in making our first session together an informative and efficient one.

Step 1- Intake Information Document: Please print this document and fill it out completely before our first session. This will make our first session together more efficient and effective. 

Step 2- Client Information Document: This document tells you about my practice, payment policies, confidentiality, and other general questions. You do not need to print this.

Step 3- Credit Card Authorization: Please print this form and fill it out completely. This is for billing purposes.

Step 4- Insurance Form: If you plan on using Blue Cross/ Blue Shield then print this off, fill it out, and bring it to our first session.

Step 5- Notice of Privacy Policy: This is a standard form required by federal law. Please print the first page only, sign it and bring it to our first session.

Step 6- Signature Page: Please print out this page and bring it to our first session.

Step 7- Release of Information: This form allows me to communicate with other individuals about your treatment. People such as current psychiatrist, primary care physicians, dietitians, etc. If there is no one you need me to be in contact with then you do not need to fill out this form. You can fill out this form at any point during treatment if something were to change.
